In order to protect our wildlife a background check will be run on each volunteer.

Medical Survey

It is not the intent of Vanishing Species to invade your privacy. However, as a volunteer within our organization, we must be aware of any medical history that may impact upon your services. In answering the following questions you will not necessarily be rejected as a volunteer. We simply wish to ensure your safety, as well as the safety of the wildlife we protect. Do you or have you ever had any of the following:

VANISHING SPECIES VOLUNTEER & STAFF AGREEMENT

WHEREAS, VANISHING SPECIES, INC. is a non-profit Florida Corporation engaged in the benevolent enterprise of aiding and caring for sick, abused, neglected, confiscated, abandoned, and boarded wildlife and exotic animals of all genuses and species, herein referred to as "wildlife"; and
WHEREAS, VANISHING SPECIES, INC. engages in the education of the general public about said wildlife, and
WHEREAS, VANISHING SPECIES, INC. is dependent upon the monetary contributions of its benefactors and clients, and
WHEREAS, VANISHING SPECIES, INC. is dependent upon the contributions of time given by its dedicated and concerned staff and volunteers herein referred to as "staff"; and
WHEREAS, the undersigned recognizes the inherent danger in working with or near wildlife, and recognizes that the undersigned may be endangered by said wildlife which may inflict injury, disease, allergic response, or death; and
WHEREAS, the undersigned assumes the risks involved with any work performed beyond working with wildlife, including, but not limited to lawn maintenance, heavy equipment, farm equipment, construction, preparation of food, cleaning, transport, public events, and recognizes that the undersigned may be endangered by said services which may inflict injury, disease, allergic response, or death.

NOW, THEREFORE, in consideration of these premises and of the privilege and opportunity of being permitted to volunteer, work for, or lend assistance to Vanishing Species, Inc. to further its charitable and benevolent cause and purpose, the undersigned does hereby agree as follows:

1. The above representations are true and correct and made a part hereof.
2. Acknowledgement of Inherent Danger: The undersigned has been apprised of and understands the dangers inherent in working with wildlife and related duties, and voluntarily enters into this agreement in the interest of promoting and protecting the best interests of Vanishing Species, Inc.
3. Liability Waiver: The undersigned hereby holds Vanishing Species, Inc. and/or its representatives harmless for any and all damages that the undersigned has incurred or may incur in the future as a result of performing services for Vanishing Species, Inc. and waives any and all causes of action, claims for damages, and any and all other rights of the undersigned arising from or which may result from the activities associated with performing services for Vanishing Species, Inc.
4. Copyright Waiver: The undersigned hereby acknowledges and agrees that photographs or recordings including but not limited to pictures, sound, and video, herein referred to as "media", of animals, events or any activities associated with Vanishing Species made by the undersigned are, and remain, the intellectual property of Vanishing Species, Inc. The undersigned hereby releases any claim, rights, or royalties to such media and agrees to provide copies of all such media to Vanishing Species, Inc. Vanishing Species may use such media for advertisement, merchandise or any other use at its discretion, without royalties, credits, or compensation to, or consent from the undersigned or any third parties. A credit to the undersigned may be included with the media in any distributed form at the discretion of Vanishing Species, Inc., if requested by the undersigned. Recordings, film, negatives, or other reproductions may be made and kept by the undersigned for personal use, but distribution of such media, gratis or for sale, is strictly prohibited without the expressed written consent of Vanishing Species or its authorized agents.
5. Non-Compete: The undersigned agrees to not engage in volunteer or paid work for another non-profit wildlife sanctuary, rescue, or rehabilitation center engaged in the care of wildlife or educating the public about such wildlife, excluding zoos, veterinary offices, or other such zoological medical facilities while volunteering or working for Vanishing Species, Inc., without the expressed written consent of Vanishing Species, Inc.
6. Non-Disclosure: The undersigned hereby agrees to not disseminate information including, but not limited to clients, contributors, business practices or other information about or associated with Vanishing Species, Inc. unless such information is freely available to the general public without the disclosure of such information by the undersigned.
7. Scheduling Agreement: The undersigned acknowledges that failing to honor scheduled volunteer or work commitments, or appearing without notification at facilities or events associated with Vanishing Species, Inc. is disruptive and potentially dangerous. The undersigned hereby agrees to notify an authorized agent of Vanishing Species, Inc. at least twenty-four (24) hours in advance of the undersigns intended appearance at or commitment to a facility or event associated with Vanishing Species. Exceptions will be made to this rule in only the most extreme circumstances including but not limited to: death in the immediate family, hospitalization, contagious disease, death, or natural disaster.
8. Severability: The undersigned acknowledges and agrees that Vanishing Species, Inc is a privately held company. As a result, the undersigned may be terminated as a volunteer or staff member for any violation of this agreement or any other reason at the discretion of authorized personnel of Vanishing Species without notice or appeal.
